Requesting Duplicate Diplomas
Students with lost or damaged diplomas may order replacement diplomas. Replacements for an Associate or Bachelor diploma are available for a fee of $13.00. The graduate (e.g. Master鈥檚 degree) diploma is $20.00. To request a duplicate diploma, please download and complete the appropriate Request for Replacement Diploma Form below:
Upon receipt of the order form, please allow up to four (4) weeks for delivery.
You must have installed on your computer to view and print the aforementioned form.
